This Patient Guide has been developed to provide patients, families, representatives and other interested parties with clear information about the services provided by Equinox Physiotherapy. It outlines the aims and objectives of the service, the treatments and facilities available, the terms and conditions of care, the contractual arrangements between patients and the service provider, and the process for raising concerns or complaints. The guide also explains how patient feedback is used to support service improvement and provides information regarding Healthcare Inspectorate Wales (HIW) registration and inspection. Its purpose is to ensure that patients have access to accurate, transparent and up-to-date information to support informed decision-making and promote confidence in the quality and governance of the service.
